Referring yourself
If you’re looking to self-refer online, you can do so by visiting URefer www.urefer.co.uk/workwell or by calling 03333 584605. A member of the WorkWell Suffolk team will review the referral and contact the participant directly.
Refer someone to WorkWell Suffolk
WorkWell Suffolk welcomes referrals from a wide range of professionals and organisations. If an individual meets the eligibility criteria and has given their consent, you can refer them for support.
WorkWell provides personalised support for people whose physical health condition, mental health condition, disability or wellbeing issue is affecting their ability to stay in or find work. We work with individuals to understand their needs, identify practical solutions and connect them with the right support to improve their health, wellbeing and ability to work.
Referrals can be made by healthcare professionals, employers, social care services, voluntary and community organisations, employment support providers, educational settings, and other professionals working with eligible individuals. The person being referred must be aware of and consent to the referral.
Before Referring
Please ensure that the individual:
- Is aged 16 or over.
- Lives in Suffolk.
- Has a physical health condition, mental health condition, disability or wellbeing issue that is affecting their ability to stay in or find work.
- Has given their consent to be referred to WorkWell Suffolk.
- Understands that WorkWell Suffolk will contact them directly following receipt of a referral.
GPs and Healthcare Professionals
Patients can self-refer online through URefer www.urefer.co.uk/workwell or call 03333 584605.
Alternatively, please download and complete the referral form available on this page with the participant’s consent. Once completed, please email the referral form to: workwellsuffolk@ahpsuffolk-cic.nhs.uk
A member of the WorkWell Suffolk team will review the referral and contact the participant directly.
Employers
In the first instance, we encourage employers to ask employees to complete a self-referral, as this allows the individual to provide information about their circumstances directly. This can be done online through URefer www.urefer.co.uk/workwell or call 03333 584605.
Alternatively, employers can refer an employee direct by downloading and completing the attached referral form with the participant and submit it by email to: workwellsuffolk@ahpsuffolk-cic.nhs.uk. Employee consent should be sought prior to the referral. We recommend running through the referral together where possible.
Guidance: Please complete as many sections of the form as possible. Employers are not expected to provide detailed or sensitive medical information. If further information is required, a WorkWell Health Coach will contact the participant directly to discuss their needs and complete the assessment process.
Professionals and Partner Organisations
Participants should be encouraged to self-refer wherever possible, as this allows them to provide information about their circumstances directly and engage with the service from the outset.
If a participant is unable or prefers not to self-refer, professionals working in health, social care, employment support, education, voluntary and community services can make a referral on their behalf.
To make a referral, please complete the referral form available on this page and return it by email to workwellsuffolk@ahpsuffolk-cic.nhs.uk.
Please complete the form with as much information as possible with the participants consent and ideally present. Sensitive medical information can be omitted if it is not relevant to the referral. If further information is needed, a Work and Health Coach will contact the participant directly to discuss their circumstances.
Once a referral is received, a member of the WorkWell Suffolk team will review the information and contact the participant directly to discuss the support available.
